A 40-year-old driver faces charges of careless driving causing death after his vehicle collided with a freight train at a rural level crossing, resulting in his wife’s death. Haydn Thomas Mansell entered a not guilty plea during his appearance in Albany Magistrates Court.
Details of the Fatal Collision
The crash occurred in July 2025 on Redmond-Hay River Road near Redmond, about 400 kilometers south of Perth. Mansell was operating a Mitsubishi Triton when it struck a grain train at the crossing. His 29-year-old wife, Cailee Mansell, seated in the passenger position, died at the scene.
The couple’s three children, also passengers, sustained minor injuries and received medical treatment. The two train crew members escaped physical harm but underwent care for shock.
Court Hearing and Upcoming Trial
Mansell faces one count of careless driving causing death, grievous bodily harm, or bodily harm. He spoke only to confirm his not guilty plea. Prosecutors outlined a list of 21 potential witnesses, though not all are expected to testify.
The case heads to a two-day trial scheduled for October.

The incident has sparked fresh demands for better safety measures at regional level crossings. One local resident described the Redmond crossing as “horrible,” highlighting ongoing concerns over infrastructure vulnerabilities.
